Children cannot begin school without a completed Connecticut Early Childhood Health Assessment on file. All ECC families should receive a link to complete non-medical and medical forms via Brightwheel. Forms must be submitted via Brightwheel prior to Thursday, August 20 for children to begin school. **REQUIRED FOR ALL CHILDREN | Must be signed and completed by your child’s pediatrician. Parents must complete and sign Part One.
Required for children who take prescribed medications, including Benadryl, EpiPen, and Auvi-Q.
Required for children with allergies and must be completed and signed by your child’s pediatrician and/or allergist.
Grants permission for ECC staff to apply ointments to children, including but not limited to diaper cream and sunscreen.
