Children’s Forms
All ECC families should receive a link to complete non-medical and medical forms via Brightwheel. Forms must be submitted via Brightwheel prior to August 20th for children to begin school. If you are having trouble with Brightwheel, please contact us at the ECC.
Children cannot begin school without a completed Connecticut Early Childhood Health Assessment on file.

Required Medical Forms
**REQUIRED FOR ALL CHILDREN** | Must be signed and completed by your child’s pediatrician.
Parents must complete and sign Part One of the form.
Required if applicable | This form is required for children who take prescribed medications, including Benadryl, EpiPen, and Auvi-Q.
Required if applicable | This form is required for children with allergies and must be completed and signed by your child’s pediatrician and/or allergist.
Required if applicable | This form grants permission for ECC staff to apply ointments, including but not limited to diaper cream and sunscreen, to your child.
