Children’s Forms

ECC Children are required to have an updated medical form on file at the ECC. A child’s medical forms expire 12 months from the date of exam. As per CT license, forms must be submitted to the school before the expiration date.

To submit an updated medical form during the year, please e-mail 0r drop off at the school office. Children whose forms are late risk not being able to attend school.  A child’s health and safety are always our priority. If your child develops any new allergies or intolerances during the school year, please make sure to notify the school.

Required Medical Forms

Children with allergies or medications must always have updated forms and medication in order to be at school.

**REQUIRED FOR ALL CHILDREN** | Must be signed and completed by your child’s pediatrician.

Parents must complete and sign Part One of the form.

Required if applicable | This form is required for children who take prescribed medications, including Benadryl, EpiPen, and Auvi-Q.

Required if applicable | This form is required for children with allergies and must be completed and signed by your child’s pediatrician and/or allergist. 

Required if applicable | This form grants permission for ECC staff to apply ointments, including but not limited to diaper cream and sunscreen, to your child.

* If your child requires medications or ointments, our license requires that parents bring unopened, unexpired bottles with the child’s name clearly labeled
