Forms and Documents

The dropbox to submit medical forms is now closed for the school year. To submit updates to medical forms during the year, please bring them directly to the school office. A child’s medical forms expire 12 months from the date of exam. As per our CT license, forms must be submitted to the school before the expiration date. Children whose forms are late risk not being able to attend school.  For an Early Childhood Health Assessment please click here. At any point during the school year if your child develops any new allergies or intolerances please email Lindsay directly at lweiner@tiwestport.org or call 203-227-1656. In addition, if you would like to update your emergency forms during the school year, please send an email to gstone@tiwestport.org.

Required Medical Forms

If your child requires medications or ointments, unopened, unexpired bottles with your child’s name clearly labeled.

Children with allergies or medications cannot begin school without authorization by our nurse.

**REQUIRED FOR ALL CHILDREN** | This form needs to be signed and completed by your child’s pediatrician. 
Parents must complete and sign Part One of the form.

Required if applicable | This form is required for children who take prescribed medications, including Benadryl, EpiPen, and Auvi-Q.

Required if applicable | This form is required for children with allergies and must be completed and signed by your child’s pediatrician and/or allergist. 

Required if applicable | This form grants permission for ECC staff to apply ointments, including but not limited to diaper cream and sunscreen, to your child. 

Required Non-Medical Forms

This fillable PDF document will be automatically sent to the ECC. Fill out one form per child. Please note that the forms must be completed in one sitting; there is no option to save and return to them later.
